Position Title:    Clinical Care Coordinator for the Neonatal Follow Up Program and Developmental Assessment Clinic

Program summary: The Neonatal Follow Up Program is a multi-disciplinary program that provides comprehensive evaluations and follow up care to infants who are at high risk for neuro-developmental problems.   

Purpose of Position: To provide expert nursing care and care management in the outpatient setting to infants who are at high risk for complex health problems and/or delayed development as a result of prematurity, birth trauma, and/or neurological complications; to identify gaps and needs of the patient population and seek to implement interventions to improve patient outcomes;  to function as a consultant in the area of infant development to nursing and other professional staff as well as professional in the community;  to assist with the implementation of new nursing techniques and strategies; and to demonstrate clinical nursing leadership through clinic practice, staff and student education, and through participating in research.

Organization Relationships: Administratively responsible to the Clinical Nursing Director of the Newborn ICU.  Functionally responsible to the Neonatal-Perinatal physicians in the inpatient unit and to Developmental Behavioral Pediatric physicians in the ambulatory care setting.

Responsibilities:

1. Patient Care
2. Provides expert nursing care and nursing support to high risk infants and their families through individual assessment, evaluation and monitoring of progress during the first few years of life.
3. Assesses and identifies risk factors of infants in the Newborn ICU and enrolls infants meeting high risk criteria into the Neonatal Follow Up Program. 
4. Triages internal and external referrals by obtaining and reviewing patient records, consulting with referring providers, and performing patient and family assessments in advance of clinical appointments.
5. Functions as a liaison between referring institutions and health care professionals outside of Michigan Medicine and professional staff within Michigan Medicine while the infant is inpatient as well as after discharge.
6. Participates in physical, psychosocial and developmental evaluations of infants enrolled in the Developmental Assessment Clinic.
7. Utilizes evaluation data to facilitate the development, implementation and evaluation of a comprehensive plan of care that is family centered.
8. Counsels parents on bio-psycho-socio-developmental progress of their infants and assists them in making informed decisions related to their infant?s care.
9. Assists with referrals and diagnostic tests, and follows up with results. Coordinates appropriate referrals with other sub-specialists in developing appropriate interventions. 
10. Provides care management for patients with established developmental conditions in accordance with the team?s plan. Assures seamless transition to outpatient resources.
11. Problem-solves system barriers to clinical care by utilizing resources across the health system and in the patient?s local community. 
12. Responds to patient treatment-related phone calls, and in-basket messages.
13. Initiates program evaluation and facilitates program change in collaboration with other team members associated with the Neonatal Follow Up Program.  Supports standardization and development of program protocols, policies and procedures using evidence-based practice.

3. Identify current and potential staff education needs. Works with nurse educators to plan, implement and evaluate programs to increase clinical knowledge and skills of those caring for this patient population.
4. Maintain knowledge of current medical, developmental, and behavioral treatment modalities by participating in in-services, workshops, formal classes and/or conferences.
5. Participates in the learning experiences of nursing students and other health care professionals through formal and informal mechanisms.

2. Incorporates nursing research findings into clinical practice. 
3. Collaborates with nursing experts and other disciplines to establish, implement and monitor standards of care.
4. Initiates or participates in research studies and/or quality assurance activities in compliance with federal and organizational regulatory guidelines.

2. Provides clinical leadership, functional supervision, and role modeling for nursing practice.
3. Participates with other health team members in problem solving related to clinical and operational aspects of the clinic setting
4. Participated in committee/task forces to assist with planning and policy setting

Required Educational Requirement; applicant must meet one of the following:

Bachelors degree in Nursing OR

Associates degree or diploma in Nursing and a Masters degree or higher in Nursing

Three or more years of nursing experience including at least 1 year of neonatal nursing
