Clinical pharmacy services are provided throughout Michigan Medicine to a diverse array of patients and include clinical pharmacy programs that complement our drug distribution system.  Pharmacy Services have been organized to follow a service-line approach to better align pharmacy resources and provide the best health care outcomes for patients, which allows pharmacists to participate in teams that coincide with the way care is provided.  Clinical pharmacy services offered and required of this position from a departmental perspective include code blue response, pharmacokinetic monitoring and dosing for specific drugs, order review and verification, anticoagulation therapy monitoring and patient education, renal dosing, parenteral nutrition monitoring, and comprehensive profile review. In addition, pharmacists provide leadership to ensure that the organization achieves core safety and quality performance metrics related to medication management.     

Pharmacist generalists actively collaborate with the health care team in designing, implementing, and monitoring patient-specific pharmacotherapy on various surgery services. The pharmacist when covering a clinical role attends daily rounds to ensure and promote the safe, rational, efficacious, and cost-effective use of drug therapy. This will include activities such as review of medication orders, provide dosage adjustments per pharmacy policies for anticoagulation, pharmacokinetic antimicrobials, renally dosed medications, parenteral nutrition, and conduct daily profile reviews.  The development of guidelines and policies, quality improvement processes, research, and assistance in the clinical training of staff, residents and students is also expected. This position offers the opportunity to serve as a preceptor for University of Michigan College of Pharmacy Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs) and obtain adjunct faculty status within the College.  Additionally, there are many opportunities to mentor and facilitate the growth and development of pharmacy residents through our strong post-graduate pharmacy education programs.

This will be a full time (1.0 FTE) position with dayshifts and some evening shifts.  The position will cover a variety of clinical surgery services including but not limited to urology, plastics, orthopedics, oral maxillofacial, otolaryngology, general surgery, and thoracic surgery.   This position will also rotate in operational areas, such as the main central pharmacy, OR pharmacy, cleanroom, etc. Training will depend on departmental needs, as well as team member interest.  The position is designed to provide weekend coverage on a rotating basis.  Weekend expectations are reviewed on an annual basis and subject to change based on departmental needs.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES:

• After successful orientation and training, the pharmacist will cover clinical surgery services as well as operational roles. The Pharmacy Department offers services 24 hours a day, 7 days a week, 365 days a year. This position will include daytime hours as well as rotate into some evening shifts.
• Specific responsibilities may include (but are not limited to) the following:
  • Service Based, Team Pharmacist: Activities include interdisciplinary rounding, comprehensive profile reviews, pharmacokinetic monitoring and dosing, anticoagulation therapy monitoring and dosing, discharge counseling as needed, renal dosing, parenteral nutrition monitoring and adjustments, pharmacy leaner precepting, and education of multidisciplinary team members.
  • Participate in Michigan Medicine committee(s)
  • Lead drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with Health Systems needs and objectives 
  • Prioritize daily responsibilities with accuracy and dependability 
  • Demonstrate and apply excellent teamwork and communication skills
  • Effectively communicate with pharmacy team members, other healthcare professionals, employees, patients and visitors
  • Active membership and leadership in professional organizations
  • Accurately and efficiently interpret and verify medication orders
  • Code Blue coverage on a rotating basis

Teaching

• Pharmacists serve as preceptors for the University of Michigan College of Pharmacy students, for both Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs). Pharmacists are expected to mentor and precept pharmacy residents within the Department's post-graduate educational programs.

Quality Improvement

• Develop and implement quality improvement measures and process improvements to improve patient care and pharmacy services

SUPERVISION RECEIVED:

Functional supervision is received from a Pharmacy Manager/Assistant Director, or Pharmacy Manager/Assistant Director designee. Administrative supervision is received from a Pharmacy Manager/Assistant Director, or other designated official.

• Doctor of Pharmacy Degree from an ACPE-accredited College of Pharmacy or equivalent combination of education and experience
• Completion of an ASHP-accredited PGY1 residency or Equivalent combination of education, training and clinical experience
• Current licensure as a registered pharmacist by the State of Michigan Board of Pharmacy 
• Michigan Controlled Substance License to practice pharmacy in the State of Michigan, or eligibility to become licensed within 60 days of employment start date
• ACLS certification - can be obtained once hired through Michigan Medicine
• Other: Ability to perform a variety of manual dexterity movements, including keyboarding and active participation in codes (e.g. sterile product preparation). Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook).  Fluency in reading, writing, and speaking English.
• Ability to meet all the above qualifications and responsibilities with or without reasonable accommodation

• Board-Certified Pharmacotherapy Specialist (BCPS) or plans to obtain once eligible 
• Clinical pharmacy, including surgery, experience.
• Recent inpatient clinical hospital practice experience including participation in multi-displinary rounds, direct patient care and education, in patient populations similar to those at an academic medical center.
