Why Join the Department of Social Work and Adult Neurorehabilitation/PM&R team?
This Social Worker would provide psychosocial support, care management, and care coordination for adult patients across the outpatient neurorehabilitation sites (Briarwood Building 04/MedRehab, Brighton Center for Specialty Care, and Canton Health Center) as well as the Physical Medicine & Rehabilitation (PM&R) Clinic at the Burlington Building. The patient population includes adults recovering from traumatic brain injuries, spinal cord injuries, stroke, sports-related injuries, burns, falls, mobility concerns, and other related conditions.
As a member of these teams, you will play a vital role in providing psychosocial services that encompass accurate assessment of mental health needs, support/counseling, social work interventions, and care coordination for a medically complex patient population. Additionally, this role will provide crisis intervention, safety planning, case management services and community referrals, as well as patient education, group support, and facilitation of multi-disciplinary team meetings. This role is a vital support to the Adult Neurorehabilitation and PM&R patients and medical teams, crucial to maximizing access to care and ensuring continuity of care after patients discharge from inpatient rehabilitation.
This position also includes non-clinical, operational, and programmatic responsibilities to support referral management, interdisciplinary collaboration, process improvement initiatives, and patient engagement efforts across Adult Neurorehabilitation. Because this is a unique and evolving role, these responsibilities may begin at approximately 10hr/week and grow and adapt over time in response to program needs, emerging patient care priorities, and opportunities for innovation and service development.
