This Social Worker would provide psychosocial support, care management, and care coordination for adult patients across the outpatient neurorehabilitation sites (Briarwood Building 04/MedRehab, Brighton Center for Specialty Care, and Canton Health Center) as well as the Physical Medicine & Rehabilitation (PM&R) Clinic at the Burlington Building. The patient population includes adults recovering from traumatic brain injuries, spinal cord injuries, stroke, sports-related injuries, burns, falls, mobility concerns, and other related conditions.

As a member of these teams, you will play a vital role in providing psychosocial services that encompass accurate assessment of mental health needs, support/counseling, social work interventions, and care coordination for a medically complex patient population. Additionally, this role will provide crisis intervention, safety planning, case management services and community referrals, as well as patient education, group support, and facilitation of multi-disciplinary team meetings. This role is a vital support to the Adult Neurorehabilitation and PM&R patients and medical teams, crucial to maximizing access to care and ensuring continuity of care after patients discharge from inpatient rehabilitation. 

This position also includes non-clinical, operational, and programmatic responsibilities to support referral management, interdisciplinary collaboration, process improvement initiatives, and patient engagement efforts across Adult Neurorehabilitation. Because this is a unique and evolving role, these responsibilities may begin at approximately 10hr/week and grow and adapt over time in response to program needs, emerging patient care priorities, and opportunities for innovation and service development. • Provides regular and planned cross-coverage for Primary Care and Specialty Care pager services for urgent needs while serving as Social Worker of the Day, as well as for routine needs for Unallocated Specialty Care Clinics as assigned.

• Working directly with patients both face-to-face and virtually by phone or video visits. 

• Respond to patient and staff inquiries with professionalism, accuracy, and confidentiality.

• Support a culture of teamwork, respect, inclusion, and patient-centered care.

• Referral monitoring and management

• Support interdisciplinary referral coordination, and documentation as appropriate.

• Serve as a liaison among patients, families, providers, clinical staff, and administrative teams

• Participate in opportunities to improve clinic workflows, process improvements, and patient service.

• A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
• A current license to practice social work in the State of Michigan (LMSW or LLMSW).
• Previous experience with medical and mental health social work.
• Previous experience working with conditions related to traumatic brain injuries, spinal cord injuries, stroke, sports-related injuries, burns, falls, mobility concerns, and other related conditions. 

• Skilled in developing rapport and effectively communicating with a diverse patient population.
• Proficiency in adjustment counseling, grief/loss, crisis intervention, suicidality and coping issues. 
• Skilled in utilizing mental health screening tools such as C-SSRS, PHQ-9, GAD-7
• Skilled in utilizing therapeutic modalities including CBT, DBT, ACT, MI, Mindfulness
• Efficient, accurate psychosocial assessment of patient needs and effective case dispositions.
• Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical setting.
• Independent, sound decision-making is necessary for success in this position. 
• Effective collaboration with large multi-disciplinary teams and facilitation of rounds.
• Proficiency in keeping accurate and timely clinical records by computer.
• Excellent interpersonal, written, and verbal communication skills.
• Strong organizational skills and attention to detail.

This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00. 

This position is expected to be hybrid, with remote work and required onsite presence at designated locations. The primary sites for this position are Briarwood, Canton, and Brighton Specialty. The employee will be start with onsite two days per week and remote three days per week but may increase in the future. Current onsite work will include weekly presence at Briarwood and alternating weekly coverage between Canton and Brighton Specialty. The Physical Medicine & Rehabilitation Clinic will likely require planned onsite presence for patient care and/or team needs in the future.

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.  Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification

This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
