As a member of the Ann Arbor Meals on Wheels (AAMOW) team, this individual will play a vital role in protecting the health and quality of life for the individuals that are served through our program. The individual in this role will be responsible for assessing clients seeking and already receiving services from Ann Arbor Meals on Wheels in order to ensure eligibility for home delivered meals and to provide referrals, as needed, to support clients in their home environments. The MSW Social Worker conducts in-home biopsychosocial and nutritional risk assessments, provides ongoing case management support, as needed, and assists in oversight of student interns and office assistants. To be successful in this role, candidates should have a passion for working with older adults and individuals with disabilities in the community and possess strong critical thinking, organizational, and problem-solving skills. We are looking for candidates with exceptional interpersonal skills who can successfully work within a dynamic, collaborative team. 

Clients
?    Conduct in-home psychosocial and nutritional risk assessments and reassessments with clients to determine program eligibility and make referrals to appropriate community resources
?    Identify gaps in services and areas of need for clients served by AAMOW
?    Provide case management, crisis intervention, comprehensive support, referrals, and appropriate advocacy for clients
?    Develop and coordinate special projects to assist clients in maintaining independence in their homes and helping them age in place
?    Coordinate with and provide coverage for AAMOW's BSW Social Worker, including following up on client inquiries, scheduling and conducting intakes, faxing and tracking required client               forms, and sending client letters

Relationship-Building
?    Establish, foster, and strengthen relationships with community partners
?    Create and strengthen relationships with senior living community service coordinators, complex care managers, nurse care navigators, and other healthcare providers
?    Represent AAMOW at community events and participate in various collaborations and partnerships on behalf of AAMOW, including, but not limited to, Barrier Busters and the Senior Crisis          Intervention Program (SCIP)
?    Collaborate with other health, mental health, and allied health professionals in delivering services to homebound adults
?    Facilitate educational sessions for the AAMOW team and community members on relevant topics impacting our clients
?    Co-lead monthly meetings with AAMOW's BSW Social Worker, Clients Services Coordinators, and Client Care Associates to case consult and support client services

Supervision
?    Provide functional supervision and training for student interns and office assistants under the oversight of the AAMOW Director

Administrative
?    Manage reassessment and assessment schedules in accordance with grant requirements and in collaboration with AAMOW?s BSW Social Worker
?    Engage in quality improvement activities 
?    Maintain electronic and paper client files 
?    Keep social work office organized, including maintenance of community resources

Other Related Duties
?    Assist with meal deliveries utilizing personal vehicle or University-owned van, provide front office coverage, and help prepare the office for volunteer arrivals and returns, as needed
?    Assist with reporting, fundraising activities, grant applications/funding proposals, and program design, as needed
?    Perform other related duties as assigned



 

• A Master's degree in social work from a school accredited by the Council of Social Work Education
• Some experience of professional social work
• Current license to practice social work in the State of Michigan (LMSW or LLMSW)
• This classification requires Primary Source Verification
• Experience conducting biopsychosocial assessments
• Experience in direct service work with diverse groups
• Highly proficient with Microsoft Office
• Valid driver's license and ability to drive personal and U-M vehicles within service area

• Minimum of two years of experience working with older adults and/or individuals with disabilities
• Minimum of one year of experience conducting home visits and in-home assessments
• Skilled in multitasking, prioritizing, outreach, networking 
• Ability to present effectively to large and small groups
• Experience supervising interns and student workers
• Strong knowledge of resources available locally/statewide/nationally
• MiChart and client database experience

This is full-time, 40 hour/week position. 

The typical schedule is Monday-Friday from 8:30am-4:30pm along with one Saturday per month (which includes a flex day during that work week). It also includes coverage on rotating holidays.

This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
