The Interventional Nephrology Clinical Care Coordinator (CCC) serves as a key member of the multidisciplinary Interventional Nephrology team, providing comprehensive nursing coordination and clinical support for patients with acute and chronic kidney disease who require vascular access procedures and ongoing nephrology interventions. This role serves as a central point of contact for patients, families, and care teams, providing education, addressing clinical questions, and ensuring continuity of care. The coordinator identifies gaps in care, supports quality improvement initiatives, and partners with internal and external stakeholders to standardize processes and optimize patient outcomes.

The ideal candidate demonstrates strong clinical judgment, exceptional organizational and communication skills, excellent attendance, proficient computer skills and the ability to manage multiple priorities in a fast-paced, dynamic environment while maintaining a high standard of professional practice and patient-centered care.

Patient Care and Coordination

• Coordinate care for patients undergoing interventional nephrology procedures, including dialysis access creation, maintenance, and interventions. 
• Coordinate, plan, and facilitate referrals for renal biopsies, tunneled catheters, prolines, and other vascular access devices. 
• Participate in phases of patient care including pre-procedural, post-procedural procedure recovery, and cardiac monitoring. 
• Participate in patient care, teach and educate patients in the Nephrology Access Clinic
• Maintain multiple referral databases to be used by various members of the health care team for research projects as well as contribute to the quality assurance program designed to identify and evaluate access creation in accordance with KDOQI guidelines. 
• Utilize nursing process to provide nursing care to facilitate and maintain optimal level of health for patients and families
• Provide instruction and consultation to members of the health care team, collaborate with other healthcare team members by utilizing associate resources to provide continuity in patient care and participate with health team members for problem solving. 
• Maintain patient referrals, scheduling, pre-procedure preparation, and post-procedure follow-up for the inpatient and outpatient patient type.
• Ensure appropriate diagnostic testing; laboratory studies, and clinical documentation are completed prior to procedures. 
• Collaborate with nephrologists, advanced practice providers, nurses, radiology staff, dialysis centers, and referring providers to facilitate seamless patient care. 
• Monitor patient progress and address barriers to care to improve access and outcomes. 

Clinical Assessment and Triage

• Perform telephone triage and clinical assessments for patients experiencing access-related concerns or procedure-related symptoms. 
• Prioritize patient needs and coordinate urgent evaluations when indicated. 
• Review patient histories, medications, laboratory results, and clinical information to identify potential concerns and communicate findings to providers. 
• Provide clinical recommendations within the scope of practice and established protocols. 

Patient Education

• Educate patients and families regarding procedures, vascular access care, treatment plans, medications, and follow-up requirements. 
• Reinforce provider instructions and assess patient understanding. 
• Develop and maintain educational materials that support patient engagement and self-management. 

Care Management and Follow-Up

• Track patient outcomes and ensure completion of recommended follow-up appointments, procedures, and treatments. 
• Coordinate care transitions between inpatient, outpatient, dialysis, and specialty care settings. 
• Monitor procedure results and communicate findings to patients and referring providers as directed. 

Communication and Collaboration

• Serve as a liaison between patients, referring providers, dialysis units, nephrologists, APPs, radiology teams, and ancillary services. 
• Facilitate multidisciplinary communication to support timely decision-making and continuity of care. 
• Participate in patient care conferences, team meetings, and quality improvement initiatives as appropriate. 
• Responds to emails and in basket messages from physicians, advanced practice providers, clerks, nurses, and referring providers, including new referrals. 

Documentation and Regulatory Compliance

• Document patient interactions, assessments, care plans, and coordination activities in the electronic medical record. 
• Ensure compliance with institutional policies, regulatory requirements, and professional standards. 
• Maintain patient confidentiality and adhere to HIPAA requirements. 

Quality Improvement and Program Development

• Participate in quality improvement initiatives aimed at improving patient outcomes, access to care, and operational efficiency. 
• Assist with data collection, reporting, and monitoring program metrics. 
• Identify opportunities to enhance workflows and patient experience. 
• Other duties as assigned both clinical and non-clinical duties 

• Current licensure as a Registered Nurse in the State of Michigan
• Bachelor of Science Degree in Nursing OR associate's degree/Diploma in Nursing along with a master's degree in nursing.
• Minimum 5 out of the last 7 years of hemodialysis and end stage renal disease nursing experience in an acute or chronic dialysis setting.
• 2 years of recent (within the previous 5 years) experience in a clinical leadership role such as charge nurse, preceptor or similar role demonstrating organizational, prioritization and multi-tasking skills.
• Effective communication and problem-solving skills demonstrated by employment references and feedback from others and performance evaluations. 
• Self-directed and motivated to work independently and collaboratively with other health care providers as demonstrated by employment references and feedback from others and performance evaluations.
• Ability to manage multiple tasks simultaneously.
• ACLS, BLS and CAC Certifications 
• Previous work performance demonstrates self-direction, autonomy, and flexibility. 
• Strong leadership skills as evidenced by previous evaluation documentation.
• Previous work performance demonstrating excellent critical thinking and problem-solving skills
• Demonstrated expert communication skills as evidenced by interviews, references, and performance evaluations.
• Participation in unit activities that demonstrate the ability to work collaboratively with and positively contribute to a team approach to patient care.
• Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references, and performance evaluations.
