The Interventional Nephrology Clinical Care Coordinator (CCC) serves as a key member of the multidisciplinary Interventional Nephrology team, providing comprehensive nursing coordination and clinical support for patients with acute and chronic kidney disease who require vascular access procedures and ongoing nephrology interventions. This role serves as a central point of contact for patients, families, and care teams, providing education, addressing clinical questions, and ensuring continuity of care. The coordinator identifies gaps in care, supports quality improvement initiatives, and partners with internal and external stakeholders to standardize processes and optimize patient outcomes.
