The Clinical Care Coordinator for Cross Sectional Interventional Radiology (CSIR) provides comprehensive nursing coordination and clinical support for patients undergoing diagnostic and therapeutic image-guided procedures across the care continuum. Working within a highly collaborative, multidisciplinary environment, this role supports Cross Sectional interventional radiologists, advanced practice providers, nursing staff, and referring providers in the management of complex acute and chronic conditions.
The CSIR Clinical Care Coordinator is responsible for facilitating efficient patient throughput by coordinating pre-procedure evaluation and communication, procedural readiness, and post-procedure follow-up for both inpatient and outpatient populations.
This role serves as a central point of contact for patients, families, and care teams, providing education, addressing clinical questions, and ensuring continuity of care. The coordinator identifies gaps in care, supports quality improvement initiatives, and partners with internal and external stakeholders to standardize processes and optimize patient outcomes.
