Responsibilities*

 
  • Provide expert nursing care to Endocrine patients in the outpatient clinic. 
  • Assess learning needs of patients/families. 
  • Develop individualized teaching plan with patients/families. 
  • Complete 1:1 sessions with patients/families
  • Conduct education group classes for patients with diabetes, including in-person and virtual settings.
  • Provide outpatient clinic visits in-person or virtually. 
  • See patients with new onset diabetes as well as established patients in the Adult Diabetes Education Program. 
  • Place appropriate referrals for patients and families as indicated.
  • Communicate with program staff and instructors through the permanent education record. 
  • Evaluate outcomes/progress frequently during the teaching process. 
  • Charting and education must meet national and state standards or diabetes education to meet billing requirements. 
  • Cross cover for other RN diabetes educators. 
  • Participate as a member of the program advisory committee.
  • Assist with annual curriculum review and formal program evaluation. 
  • Maintain professional competencies. 
  • Insulin pump training/intensive insulin management. 
  • Functional supervision of unlicensed assistive personnel. 
  • Motivational coaching for patients/families.  
  • Assist in orientation of team members as appropriate, including observation and participation of patient care activities. 
  • Actively participate and support performance improvement activities related to self, clinic, department, and institutional goals and objectives. 
  • Other responsibilities as assigned.

Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework LEVEL C.
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level F.

Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications*

 
  • Current license to practice nursing in the State of Michigan (RN).
  • Three (3) years in the past 5 years of RN experience, caring for adult Endocrine patients. 
  • Current CDCES (Certified Diabetes Care and Education Specialist) or BC-ADM (Board Certified in Advanced Diabetes Management) or must agree to obtain the above certification within one (1) year of appointment to this position.
  • One (1) year of experience with a variety of glucometers
  • One (1) year of experience providing group education to patients and/or families
  • Demonstrated excellent verbal and written communication skills as evidenced by interviews, evaluations, and reference.
  • Proficiency with MiChart inbasket management and documentation of patient encounters in electronic health record, e.g., Epic as demonstrated by interviews, evaluations, and references.

Desired Qualifications*

 
  • BSN
  • One (1) year as a certified insulin pump trainer
  • One (1) year within the past 5 years of RN experience working with patients on insulin pump therapy and intensive insulin management.
  • One (1) year within the past 5 years reviewing blood glucose records and adjusting insulin doses. 
  • One (1) year within the past 4 years of experience with Continuous Glucose Monitoring (CGM) systems (initiating and reviewing data).
  • Experience in telehealth nursing. 

Work Schedule

 
  • 36 hours/week.
  • Shift/Hours/Days: Day Shift; Mon-Fri with flexibility in schedule to meet clinic needs.  Some evenings and Saturdays may be required (nontraditional including further weekends/holidays commitment). Hours may be variable based on patient and volume needs, with flexibility required.
  • Location-Position is on-site and located at Domino's Farms MEND Clinic.  May be required to work at other locations where MEND is located, subject to change based on needs.

All new employees will be expected to float to a designated unit(s) in times of low census.
