Responsibilities
Professional RN with clinical knowledge specific to the multidisciplinary care of breast oncology, who offers individualized assistance to patients, families, and caregivers to help overcome health system barriers throughout the trajectory of their care.
Provides education and emotional support to patients and their families to help reduce the anxiety that a new diagnosis of cancer can bring.
Communicate with patients, families, and caregivers at identified touchpoints in their care (i.e.: new patient appointment, discharge from hospital, chemotherapy follow up and transitions in care)
Encourage them to be an active member of their own healthcare team and help them to make important, informed decisions regarding their healthcare.
Assist patient in overcoming any psychosocial or practical barriers ? such as lack of transportation, financial concerns, or general skepticism of the healthcare system
Promote seamless and timely patient-centered care from the time of diagnoses, through the continuum of their care. (neoadjuvant, adjuvant, preoperative and post operative treatment plans).
Provide guidance to promote positive outcomes and patient satisfaction.
Provide culturally sensitive, age-specific team-based care models in delivering nursing care for the multidisciplinary care of breast oncology patient population.
Accurate and efficient documentation of nursing care.
Knowledge and appropriate use of internal and external resources.
Support clinical triage and new referrals to facilitate appropriate scheduling, patient needs and timely access to care in collaboration with the intake coordinators.
Integral member of the multidisciplinary team including tumor board discussions and follow up.
Translate complex medical information into language patients, families, and caregivers can understand and apply.
Monitors the patient, family, and caregiver understanding and compliance with the plan of care.
Assesses and provides referrals for patients who have tangible barriers to care such as finances transportation needs and prescription costs.
Participates in the tracking and monitoring of metrics and outcomes, in collaboration with clinical administration to document and evaluate program outcomes.
Serve as an essential liaison between patients and all other care providers.
Collaborates with lay navigators to facilitate appointments for consults and support services within established service standards including social workers, dieticians, physicians etc.
Develop patient education programs and tools.
Responsible for outreach efforts to establish and maintain positive working relationships with key members of the healthcare team and community resource groups.

Demonstrated knowledge and skills providing patient and family education and reinforcement related to oncologic conditions including, but not limited to, chemotherapy, immunotherapy, oral therapy, infusion, etc.
Proficiency with typing and electronic health record documentation.
Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions.
Willingness to adapt to different populations within the team as patient demands require. Orientation and training would be provided for new subspecialties.
