The position is aligned with the Health and Independent Living Coaching Program (formerly HealthyLifetime?) research program, which is a 3-month personalized health coaching program for adults, 30 years of age or older, that includes a comprehensive assessment made through surveys and a virtual home assessment, a goal-directed strengthening action plan developed by the participant with the guidance of the health coach, and a tailored daily planning guide to enhance action steps and goal attainment. The program consists of a 12-week "intensive" period which includes five coaching sessions with the coach (2.5 hours total), along with chat sessions initiated by the participant. All sessions are done virtually using video technology via a computer.
The purpose of the research study is to assess changes in participants' health outcomes after the 12-week intensive phase. Additionally, the investigative team evaluates the relationship of level of particpant program engagement [e.g. number of video sessions attended] to magnitude of change in outcome measures.
POSITION PURPOSE AND RESPONSIBILITY
The Health Coach promotes wellness, resiliency, and independence by guiding clients to establish their vision for their long-term health and independence, identify lifestyle changes that support their vision, establish goals and strategies for healthier lifestyle behaviors considerate of their unique life circumstances, and strengthen their motivation, agency, and confidence to achieve their goals. The Health Coach will be responsible for delivering the coaching program to their assigned clients.
PRINCIPAL RESPONSIBILITIES
The Health Coach:
- Follows all study protocols and procedures.
- Elicits the client's personal health narrative and conducts an initial virtual home visit with each client and completes a comprehensive assessment which includes review of the client's Personal Health Survey and Medical and Medication History, and completion of the Center for Disease Control's Home Environment Assessment.
- Guides the client in discovery of preferences, goals, strengths and struggles as the client establishes, client-driven small step goals and action steps to develop the client's Personal Strengthening Plan through a video planning sessions, using motivational interviewing skills to evoke change talk.
- Assists the client in building capacity for self-care, problem solving and decision-making through sharing of knowledge, expertise, and custom resources by conducting six weekly video coaching sessions with each client. Assists the client in patterns recognition, insight, and correction of cognitive errors to support movement to their desired health goals.
- Summarizes each coaching session in a SOAP note reflecting on the client's story, their progress, lessons learned, and growth over the course of the program. Notes the client's response to coaching strategies, such as the importance they place on their behavioral change and their confidence that they can achieve it. Outlines strategies to try next session or missed opportunities for follow-up.
- Collects, documents and stores all required data from each client encounter.
- Actively particpates in bi-weekly clinical case consultation sessions. Uses case presentations to reflect on their coaching practice and receives input from the investigative team to actively advance their coaching competence.
